Notice of Availability of IRS Form 1095

In accordance with IRS reporting requirements, WNESU is providing this notice
regarding the availability of Form 1095-C for the 2025 calendar year. We will not
automatically mail these forms to all eligible individuals. However, you may obtain a
copy of your Form 1095-C upon request.

To request your Form 1095-C, please notify WNESU using one of the following
methods:

Request deadline & delivery:

If you request a copy of your Form 1095, we will furnish it to you within 30 days of your
request or by January 31, 2026 whichever is later. The form may be provided
electronically if you consent to electronic delivery.

This notice is posted to satisfy the IRS alternative furnishing method and will remain
available through October 15, 2026.
